Reports/complaints

    Incident Report

    ​Incident reports should be filed whenever something occurs that requires a formal report. This could be due to injury, inappropriate behavior, equipment failure, communications issue, etc. These reports go directly to the Company Manager (Beth Lewis) and are kept confidential. We will close the loop with you if you would like follow up and in that case we ask that you provide your contact information.
    The date of the incident
    Name of the event where the incident occurred
    Location where the incident occurred
    Your position in the event (Audience / Actor / Staff / etc)
    Describe the incident you wish to report in full detail.
    Your name (not required if you wish to remain anonymous).
    Your email for follow up (not required).
